Flowchart for extubation during extracorporeal membrane oxygenation (ECMO) in severe acute respiratory distress syndrome (ARDS). Following initial patient stabilization and optimization of ECMO support, conscious sedation was pursued in the absence of circulatory shock, significant bleeding, or acute brain injury. A daily assessment of clinical and gas exchange criteria based on an Extracorporeal Life Support Organization guideline [11] was then performed, with extubation during ECMO after validation by a dedicated intensive care medicine specialist.
